GP CHRONIC CONDITION MANAGEMENT PLAN (CCM)

The Chronic Condition Management Plan (CCM) provides Medicare rebates for allied health services, for people who have a chronic medical condition. You may be eligible to claim a rebate through Medicare for these services if you have a chronic condition that is being managed by your GP under a CCM plan. A chronic medical condition is one that has been or will be present for at least 6 months.

The CCM provides a maximum of 5 total rebates (valid for 18 months from the first date of service) for all Allied Health services. This means that if you are accessing more than one allied health service you are entitled 5 rebates across all services combined, not 5 rebates per service.

Your GP will decide if you are eligible for these services and will refer you where appropriate. For further information. please contact your GP.

 

You will need to provide us with your referral from your GP. Once you have given us the referral you will be eligible to claim up to a maximum of 5 rebated sessions (valid for 18 months), provided that your GP has registered your plan through Medicare.

COMPLEX NEURODEVELOPMENTAL DISORDER AND DISABILITY SERVICES (MBS M10 Group)

The existing M10 group - Complex Neurodevelopmental Disorder and Disability Services Medicare rebate has now been extended to include the following conditions:

​

- Stuttering

- Speech sound disorders including articulation disorder, phonological disorder, childhood apraxia and dysarthria.

- Cleft lip and/or palate.

​

The M10 provides a rebate for assessment sessions and 20 treatment sessions per lifetime for all Allied Health services if you are suspected or diagnosed with any of the above eligible conditions. This is only available up until a person turns 25 years old.

Your GP will decide if you are eligible for these services and will refer you where appropriate. For further information. please contact your GP.

 

You will need to provide us with your referral from your GP. Referrals are valid for 24 months.
